Chronic Kidney Disease and Home Healthcare Services
NUTRITION MANAGEMENT
DirectMed works with registered dieticians so they can educate the patients and their caregivers about good nutrition. We supply detailed shopping lists to aid in making good choices at the grocery store. We are available for any nutritional questions or concerns.
SPECIALIZED PLAN OF CARE
The plan of care begins with an extended individualized initial assessment that covers medical, physical, psychosocial, and family/community assistance. The patient and caregiver are included in every part of the process and encouraged to become active participants in their treatment team by setting and achieving short term and long term goals.
PHYSICIAN CARE SERVICES
DirectMed can provide one of the Home Care Physicians that we work with to conduct home visits upon patient request. You will NOT be required to change your Primary Care Physician if you already have one.
MEDICATION MANAGEMENT
DirectMed knows that many health issues require medications. Our staff is knowledgeable and available to educate patients and caregivers about specific details of each medication. We also teach about possible drug interactions, side effects, and any possible food/drug interaction. Depending on a patient’s unique situation, we can provide verbal and written explanations and instructions. As with all DirectMed services, we are available by telephone 24 hours a day, 7 days a week to assist with any medication issues. DirectMed has developed a medication card for the patient to keep with them. Along with updated medication information, individual notes and staff names are also included.
