Job Description: To coordinate the overall plan of care for a patient from admission to discharge in a home care environment. Must be dedicated to helping patients reach the highest possible positive outcome in a trusting and professional manner. Our goals include both better patient outcomes and decreased preventable re-hospitalizations.
We currently have needs in the Far South Suburbs Beecher, Manteno, Monee etc AS WELL AS Joliet, and other western suburban areas
Visit Rate: Start of Care =$80 ($100 if done on a weekend or holiday), $50 for a regular visit, $60 for a recert/resumption (Higher visit rates when a nurse learns our dialysis program)
Required Education: Degree from accredited school of nursing. BSN is preferred
Required Experience: At least 1 year of experience as an RN or LPN

We provide a scope of service the includes, but not limited to, diagnosis such as: Diabetes, CHF, HTN, ESRD and provide specialized training focused on patients that are receiving Outpatient Dialysis care. Our service population includes the city of Chicago and most of the surrounding suburbs.
