Pre and Post Dialysis Home Care Services
PRE AND POST DIALYSIS CARE SERVICES AT HOME
DirectMed knows that when a patient finishes with dialysis, the day is not over for them. DirectMed goes beyond traditional home health models, and looks at the person as a whole. We want the patient to be as independent and mentally healthy as possible. One of the components of our program screens for minor to moderate depression then gives the patient the tools needed to regain control of their life through behavioral and social activities specific to their interests and lifestyles.
SPECIALIZED PLAN OF CARE
The plan of care begins with an extended individualized initial assessment that covers medical, physical, psychosocial, and family/community assistance needs. This screening tool incorporates the PHQ-9. The patient and caregiver are included in every part of the process and encouraged to become active members of their treatment team by setting and achieving short term and long term goals.
PRIMARY PHYSICIAN CARE
If the patient and staff at the dialysis center prefer, DirectMed can provide one of the Primary Home Care Physicians that we work with so that primary care needs no longer have to be done by the nephrologists and their staff. This will be done ONLY if this is what the patient prefers. With the patient’s consent, DirectMed will provide the plan of care to the dialysis center to promote efficient care, supporting a team approach.
DEPRESSION MANAGEMENT
DirectMed has implemented a tool (the PHQ-9) to screen for minor to moderate depression for the dialysis patient. The program is implemented in accordance to the patient’s schedule and in the comfort of their home. By executing a proven and evidence based tool, we are trained to teach the patient problem solving skills to assist in activities of daily living. This is a “real world, down to earth approach” that can be learned by people with a variety of dialysis issues. It also teaches the patient skills to assist them in returning to prior interests and recreation skills by overcoming perceived barriers.
MEDICATION MANAGEMENT
DirectMed knows that with dialysis comes medications. Our staff is knowledgeable and available to educate patients and caregivers about specific details of each medication. We also teach about possible drug interactions, side effects, and any possible food/drug interaction. Depending on a patient’s unique situation, we can provide verbal and written explanations and instructions. As with all DirectMed services, we are available by telephone 24 hours a day, 7 days a week to assist with any medication issues. DirectMed has developed a medication card for the patient to keep with them. Along with updated medication information, individual notes, and staff names are also included.
NUTRITION MANAGEMENT
DirectMed works with registered dieticians to educate the dialysis patient and their caregivers about good nutrition. Instead of saying, “No you can’t eat that”, DirectMed assists with teaching the patient “Don’t give it upjust eat it in moderation.” We supply detailed shopping lists to assist in making good choices at the grocery store and are available for any nutritional questions or concerns.
